End of Life Planning and Consultations

Advanced Directives & Health Care Decisions

Advance directives are legal documents that allow you to spell out your decisions about end-of-life care ahead of time. It is one of the clearest ways to tell your family, friends and health care professionals what your wishes are and it avoids confusion later on.

Our professional staff will engage you in conversation that assists you in making medical decisions about your wishes should you become too ill or hurt to express them on your own. Together we will explore your options and guide you through this process from beginning to end.

Discussing your wishes about end-of-life care is often difficult and most of us tend to avoid or ignore these conversations. Generally people believe they have time before they need to tell their loved ones their wishes and for many it is simply too hard to initiate. Our staff will work closely with you to assist you in beginning these conversations. We will explore different ways to approach the topic while teaching you how to be clear and direct.

If you’re like most you may find a face-to-face meeting too difficult. In that case, one of our professionals will sit with you and your family and help facilitate the conversation. And if your like many, you can request a video session where you can tell your family without having to sit face to face with them should that be too uncomfortable for you.

Not all essential conversations are about end-of-life care. All stages of life present with issues. Our staff are skilled in all areas and can assist you with any essential conversation.
